The emergency contact column
A roster with names and positions is a team sheet. A roster with an emergency contact beside every name is the one a coach is supposed to have in the bag, and the reason this sheet is worth printing rather than keeping on a phone.
Print it, do not rely on a screen
Pitches have no signal, phones run flat, and the person holding the information is not always the person who needs it. Paper in the kit bag has none of those failure modes.
Notes
The last column carries what does not fit anywhere else: an injury to watch, a lift home that needs arranging, a subscription still owed.
What goes in each column
- Player — and a shirt number if you use them.
- Position — or squad group for a sport that does not use fixed positions.
- Contact — the parent or carer for a junior team.
- Emergency contact — a different person from the one above, which is the whole point of having two columns.
- Notes — injuries to watch, lifts to arrange, subscriptions outstanding.
Printing it
Choose A4 or US Letter before you download and the table is laid out to fit that sheet — column widths are shares of the page, not fixed millimetres, so nothing is cut off at the right-hand edge. Alternate row shading can be turned off if you are photocopying in black and white, where a pale tint sometimes reproduces as a muddy grey. If you need more than one page, set the page count before downloading rather than printing the same PDF twice; the pages are then numbered, which matters the moment two of them get separated.

Can I change the number of rows?

Can I change the column headings?
Not on this sheet — the columns are fixed so the layout stays even and legible. The title and the header label are yours to edit.
