Conference slots are short. Filling in the first two blocks before the meeting turns ten minutes of recall into ten minutes of conversation, which is the entire point of the appointment.
A column for what the parent raised
Most templates record what the teacher said. The block for what was raised from the other side is what makes this a record of a meeting rather than a delivery.
Agreed actions with a name against them
What, who, by when. An action without a name attached is a hope, and the review date at the foot is what makes the follow-up happen rather than being intended.
Filling it in beforehand
Complete "going well" and "needs attention" the day before, with the mark book open. Ten minutes is not long enough to both recall and discuss, and the recall is the half that can be done in advance.
The block most templates leave out
"Raised by the parent or carer" exists because a conference is a two-way meeting that is usually minuted in one direction. Writing down what a parent asked for is also what makes it possible to answer them next time.
Afterwards
The actions table and the review date are the only parts anybody looks at again. Photograph the sheet, or file it where the next conference will find it — a record that cannot be located before the following meeting has done nothing.

Can I edit the section headings?
The heading at the top and the label in the corner are yours to change. The section headings are fixed, which is what keeps every copy of the sheet laid out the same way.
Will it fit on US Letter as well as A4?
Yes. Sections are measured against the paper you choose, so nothing runs off the bottom of the shorter sheet.
Can I print more than one copy at a time?
Yes — set the page count up to four and the PDF carries that many identical pages.
